By paying $1750 and participating in this HEAD TO TOE HEALING MEMBERSHIP, I understand that every educator, therapist and health coach that represents Seven Thirty Enterprises, the Dr. Reese Coaching Team and Kevin W. Reese is NOT a medical doctor or physician and is not giving any medical diagnostic or treatment procedures. 

I understand all health information and guidance given by Seven Thirty Enterprises, the Dr. Reese Coaching Team and Kevin W. Reese is based on the natural laws of Mother Earth and GOD. I understand that the Dr. Reese Coaching Team believes that my body is divinely designed to correct itself. 

I understand that this agreement is for the participation in the HEAD TO TOE HEALING MEMBERSHIP and is for educational purposes only on the subject of health matters. The information given in this program is only intended to help improve my life with nutrition changes, mindfulness techniques and possible exercises. I understand that the HEAD TO TOE HEALING MEMBERSHIP does not involve the diagnosing, treatment, or prescribing of drugs for the treatment of diseases.

I understand that nutritional changes in the HEAD TO TOE HEALING MEMBERSHIP may send my body into a state of detoxification. I understand that the ancient method of detoxification can sometimes be unpleasant and include headaches, rashes and flu-like symptoms as my accumulated wastes and toxins from years of poor eating habits, breathing polluted air and the use of legal or illegal drugs can purge from my body.

I understand that all statements and products in the HEAD TO TOE HEALING MEMBERSHIP has not been evaluated by the Food and Drug Administration. I understand that all Peace Over Pain Clinic products, therapists and coaching are not intended to diagnose, treat, cure or prevent any disease.

I understand that if I’m pregnant, nursing, taking medication, or have a medical condition, I should consult my physician before executing the HEAD TO TOE HEALING MEMBERSHIP. 

I attest that I am enrolled in the HEAD TO TOE HEALING MEMBERSHIP on my own behalf and not as an undercover agent or representative for any federal, state, or local agency on a mission of entrapment or investigation.

I attest that my communications in the HEAD TO TOE HEALING MEMBERSHIP with Kevin W. Reese and the Dr. Reese health coaches or therapists will never be shared or screen shot by me. I understand that the information contained in my dialogues, whether vocally or digitally, including attachments, are confidential and are only intended for my eyes and or the eyes of the community. The use of the information by disclosure, copying or distribution is prohibited and may be unlawful. 

I understand that if I choose to follow any advice in the HEAD TO TOE HEALING MEMBERSHIP, I do so on my own behalf based on my own beliefs. By using the website, www.drkevinreese.com, the book Peace Over Pain and following the HEAD TO TOE HEALING MEMBERSHIP, I assume total responsibility and liability for my own actions.

If I use any information to improve my health status without my medical doctors approval, I am prescribing for myself and exercising my Constitutional Rights. I fully agree to hold harmless Seven Thirty Enterprises, the Dr. Reese Coaching Team, Kevin W. Reese, Amber Williams, Levi Lymer, Karen Barr, LaShawn Roach, Tamra Sullivan, Christine Dugenio, Rachel Krause, Ryan Lidell, Sue Butzow with whom I speak in any manner. 

I assume total and all responsibility and liability for my actions and decisions. I expressly assume the risks of the HEAD TO TOE HEALING MEMBERSHIP. I therefore release Seven Thirty Enterprises, the Dr. Reese Coaching Team, Kevin W. Reese, Amber Williams, Levi Lymer, Karen Barr, LaShawn Roach, Tamra Sullivan, Christine Dugenio, Sue Butzow from all liability, damages, causes of action, allegations, suits, sums of money, claims and demands whatsoever, in law for equity, which I ever had, now have or will have in the future against Seven Thirty Enterprises, the Dr. Reese Coaching Team, Kevin W. Reese, Amber Williams, Levi Lymer, Karen Barr, LaShawn Roach, Tamra Sullivan, Christine Dugenio, Ryan Liddell, Rachel Krause, Sue Butzow arising from my past or future participation in, or otherwise with respect to, the HEAD TO TOE HEALING MEMBERSHIP.

I understand that If I voluntarily record and submit videos to Seven Thirty Enterprises, the Peace Over Pain Clinic and Kevin W. Reese, that my video may be edited and distributed as promotional material for the Peace Over Pain Clinic as well as in the future when I’m done with the HEAD TO TOE HEALING MEMBERSHIP. 

I understand that if I participate in the monthly ZOOM meetings that my part in the video could be used on social media or the website as promotional content. 

I understand that the Dr. Reese Coaching Team will NOT distribute any of my body evaluations (p-rays) or lab work without consent from me. 

I understand that my de-identified data may be used for research and educational purposes at any time. I understand that Kevin W. Reese and the Dr. Reese Coaching Team wish to reveal official studies to prove that their therapeutic approach works and that I am a part of that study by signing up with this program. This means my health journey would be revealed but my name and face would be blurred and shielded. 

I understand that with my purchase of the HEAD TO TOE HEALING MEMBERSHIP I am receiving the following:

I understand that I will be automatically charged monthly via www.drkevinreese.com. I understand that if I cancel my membership, I lose all access to the features offered above. I understand If I decide to sign up again in the future, I will have to sign another agreement form. 

I understand the cost of the HEAD TO TOE HEALING MEMBERSHIP does NOT include food, supplements, blood labs and exercise equipment. I understand this membership starts the day I sign and submit this contract. I understand that all sales are final and there is no refund available once I sign and submit this contract. 

By paying $1750, I am affirming that I am NOT making a spontaneous decision and I am making a clear, concise and educated decision for my health. 

I understand that if I become dissatisfied with the HEAD TO TOE HEALING MEMBERSHIP, I will not talk or comment negatively about the HEAD TO TOE HEALING MEMBERSHIP or Kevin Reese in public or social media. I understand that if I am dissatisfied, I will handle it behind closed doors with Seven Thirty Enterprises and the Dr. Reese Coaching Team. I understand that If I do speak negatively about the HEAD TO TOE HEALING MEMBERSHIP or Kevin Reese in public or on social media, I am susceptible to a defamation lawsuit by Seven Thirty Enterprises and the Dr. Reese Coaching Team. 

In the event that a dispute arises between Seven Thirty Enterprises, the Dr. Reese Coaching Team, Kevin W. Reese and myself with respect to the services provided pursuant to this agreement or otherwise pertaining to the relationships between us, I agree to submit to binding arbitration before the American Arbitration Association (Commercial Arbitration and Mediation Center for the Americas Mediation and Arbitration Rules). Any judgment on the award rendered by the arbitrator(s) may be entered in any court having jurisdiction thereof. Such arbitration shall be conducted by a single arbitrator. The sole remedy that can be awarded to me in the event that an award is granted in arbitration is a rerun of the program fee. Without limiting the generality of the foregoing, no award of consequential or other damages, unless specifically set forth herein, may be granted to me.

I understand that this agreement shall be construed according to the laws of the State of Connecticut. In the event that any provision of this Agreement is deemed unenforceable, the remaining portions of the Agreement shall be severed and remain in full force. 

If the terms of this Agreement are acceptable, please sign the acceptance below. By doing so, I acknowledge that: (1)I have received a copy of this letter agreement; (2)I have an opportunity to discuss the contents with Seven Thirty Enterprises, the Dr. Reese Coaching Team and Kevin W. Reese and, if desired, to have it reviewed by an attorney; and (3) I understand, accept and agree to abide by the terms hereof.

I give any educators, coaches or therapists under the Dr. Reese Coaching Team permission (all rights) to discuss openly and freely my health issues and solutions with me during the HEAD TO TOE HEALING MEMBERSHIP.

I understand that this agreement is to establish a mutual trust, relationship, and an understanding of the terms. Therefore, by my signature below, I acknowledge that I have read the above pages and agree to the terms stated.
